Preparation For Your Appointment
In order to reduce your wait time, new patients are offered the opportunity to complete these forms prior to arriving for their scheduled appointment. Once downloaded as a pdf these forms can easily be printed and completed at your convenience. This provides you with time to read and understand what is being asked at your own pace and to collect any information that you might not have with you when you visit our office. For safety, please bring copies of the completed forms with you.
New patients should arrive 30 minutes prior to their appointment in order to complete any pre-appointment paperwork, which will assist us in staying on time. In order to enter this information into your electronic medical record, we still ask that one arrive 10 minutes prior to their scheduled appointment. We ask that established patients arrive 15 minutes prior to their scheduled appointment to fill out interval history and insurance forms. Patients late for appointments may require rescheduling to another time, so as not to interfere with patients arriving on schedule.
When routine follow-up appointments are requested, this will be scheduled with your personal provider. If you have an urgent need and your provider is unavailable, you will be offered the next available appointment with any of our highly qualified providers.
Always bring your current insurance card with you to each visit.
